Heather Hokulani Porter

Heather is an experienced Family Nurse Practitioner with more than two decades of progressive clinical, leadership, and educational experience across rural, surgical, vascular, and community health settings. A graduate of the University of Hawaiʻi’s Master’s Nursing Program, Heather completed a dual pathway as a Family Nurse Practitioner and Advanced Public Health Nurse with honors, and was recognized as an ʻIkaʻu Pono Scholar for her commitment to Native Hawaiian and rural health.
Heather currently practices as a Vascular Nurse Practitioner with Pacific Vascular Institute, where she provides comprehensive care for vascular surgery patients, including diagnostic evaluation, peri-procedural management, and advanced wound care. She is board-certified as a Clinical Wound Specialist and has extensive experience in general surgery, acute care, and primary care. Her clinical background spans hospital-based care, perioperative services, telehealth, VA disability examinations, and independent practice in rural family medicine clinics.
In addition to clinical practice, Heather has served in leadership and educational roles, including Director of Clinical Operations for a federally qualified health center, CEO and Certified Nurse Aide Instructor for a rural medical training institute, adjunct nursing faculty, and clinical preceptor for undergraduate and graduate nursing students. Her work consistently emphasizes culturally competent care, workforce development, and improving access to high-quality healthcare in underserved communities.
Heather is nationally board-certified, licensed in multiple states, and actively involved in professional and community organizations supporting rural health. She brings a holistic, patient-centered approach grounded in public health principles, clinical excellence, and a deep commitment to the communities she serves.
